Ministry of Prayer - Prayer Request Form

This Prayer Request Form is similar to the written form available from the Practitioner Table in the foyer at CLF. Once filled out and submitted, it is emailed to the Director of Practitioners who will pass it along to a Practitioner for prayer. The Practitioner will will treat for your Prayer Request for one week. Please note that we cannot do Treatment for a third party without their knowledge and permission.

All Prayer Requests are held in strict confidence. Our Ministry of Prayer is conducted by the Licensed Practitioners of CLF (see list at right) and is supported by the Love offerings of those blessed by our ministry.
